Job Description:

GENERAL DESCRIPTION: Delivery ofcase management services to homeless individuals related to mental health andchemical dependency. Services includeoutreach, identification, screening, and referral to community sources toaddress their health, social and economic need. Extensive liaison work with community groups, and referral sources. Responsiblefor tracking and maintenance of appropriate records in a timely and accuratemanner. Outreach Specialist is also responsible for monitoring and expandingreferral networks with agencies and organizations. KNOWLEDGE, SKILLS AND ABILITIES: -Knowledge of substance abusetreatment and available programs -Knowledge of chemicaldependency and mental health issues -Knowledge of community andcultural aspects of substance abuse -Knowledge of case managementtechniques -Knowledge of assessment,referral and client advocacy techniques -Knowledge of computersoftware used in the completion of job responsibilities -Knowledge of resourcesavailable in the community -Ability to use agencyapproved manual-based curriculum -Ability to communicate andinteract appropriately with clients and staff verbally and in writing -Ability to apply knowledgeto client outreach, engagement, assessment, referral and advocacyresponsibilities -Ability to document requiredclient and program information and prepare reports and other documentation in an accurate, timely andlegible manner -Skillful in encouraging andfacilitating client follow through with referrals and case plans -Skillful at cultivatingtrust among members of the homeless population through consistent interaction EDUCATION AND EXPERIENCE: -Graduation from an accreditedcollege or university with a Bachelor’s degree in a work related field preferred. -Two years experience in casemanagement, substance abuse, or mental health preferred -Experience with communityoutreach and public awareness campaigns (A comparable amount of
